Qué debe incluir un consentimiento, por qué importa y una plantilla lista para adaptar a tu estudio.

The consent form (also called a disclosure and waiver) is the most important document in your studio: it records that you informed the client about the process and the risks, and that they agreed to the procedure knowingly and voluntarily. A solid form protects you in a dispute and makes your studio look far more professional.

Why it matters

  • Legal protection: if a client complains later, the signed form proves the risks were explained up front.
  • Clear expectations: the client knows exactly what healing will involve, so there is far less friction.
  • Data protection: GDPR requires you to record what data you store, for what purpose and for how long.

What to include

  • Client name, date of birth and contact details
  • Health questions: allergies, skin conditions, diabetes, blood clotting issues, infectious diseases, pregnancy / breastfeeding
  • Placement, approximate size and pigments used
  • Risk disclosure: infection, allergic reaction, scarring, imperfect results
  • Confirmation that aftercare instructions were handed over
  • Cancellation and rescheduling policy
  • Image consent for portfolio and social media (yes or no, always optional)
  • GDPR data-processing consent
  • Date and signature - in two copies, one for the client

Minors

Tattooing minors is a legally sensitive area and regulation differs by country - in many places it is restricted or outright banned. Many studios simply refuse clients under 18; others require written parental consent and the parent being present. Check the rules that apply to you, and ask for the client's age at booking time, not in the chair.

Sample - short version

I, the undersigned [name, date of birth], confirm that [studio name] explained the tattoo procedure, the possible risks (infection, allergic reaction, scarring) and the aftercare requirements in detail. I am not aware of any condition that would prevent the procedure. I request the tattoo voluntarily, at the placement indicated. I have received, understood and will follow the aftercare instructions. I consent to my personal data being processed for booking purposes. I [agree / do not agree] to images of my tattoo being published.
